Duties:
- Develops a planned schedule of social and other purposeful individual or group activities which:
- Meets the individual needs and interests of each participant.
- Encourages self-care and the resumption of normal activities or, for those who cannot realistically resume normal activities, prevents or hampers further mental and/or physical deteriorations; and
- Provides opportunities for involvement, both individual and group, in the planning and implementation of the activity program.
- Provides or supervises the provision of the activities specified in the center activity program.
- Encourages participants to take part in the activity program, specifically those suited to their individual needs.
- Utilizes the center’s transportation to provide activities in the community, as indicated by the needs and interests of the participants.
- Physically able to perform the required duties and assist frail, elderly and disabled adults, as necessary.
- Complete Activity assessments for participants
- Develop, consistent with participant plans of care, activity programs that meet individual participant needs
Position Qualifications:
- Certified by the National Certification Council for Activity Professionals (NCCAP) through any of NCAAP’s approved certification tracks as an activities director (ADC), OR
- Be a qualified therapeutic recreation specialist who has three years of experience in a social or recreational program within the last five (5) years, one of which was full-time in an activities program in a health care setting for the aged, individuals with disabilities, or special populations, OR
- Be an Occupational Therapist or occupational therapy assistant currently licensed to practice in the state of WV.
- Speaks and reads English, fluently.

Duties:
(TLC Caregiver) performs in-home services that are all non-medical services to individuals who require assistance with daily living activities. The following is a list of services that TLC Caregivers may routinely perform; however, it is not exclusive of other reasonable requests:
- Responsible for communicating all schedule changes, concerns, and any other care related issues directly to their supervisor.
- Responsible for documenting in accordance with the company documentation system set forth.
- Caregiver is expected to interact with patient and in social settings with high integrity, politeness, respect and excellent communication.

Duties:
- The LPN provides full range of hospice nursing care to patients and families in the Home Care Program under the supervision of a hospice RN as a supplement to the care provided by the RN.
- Provides personal care such as bathing
- Monitors patients’ medical conditions and provides accurate reports to attending physicians as appropriate and to the patients’ RN.
- Works in an interdisciplinary care setting to teach, assist, and support patient/family with the dying process.
- Covers On-Call as needed
- Carries out the pain management and symptom management plan to keep the patient as comfortable as possible.
- Reassesses pain and notifies the registered nurse when pain is not manageable on the current medications.
- Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
- Provides care within all service areas of Hospice of Huntington, Inc.
Minimum Qualifications:
- License to practice by the West Virginia Board of Examiners for Licensed Practical Nurses. Must also possess license in Ohio or application must be in progress.
- Minimum of 1-year patient care experience; hospice or home health preferred.
- Proof of liability insurance.

Must be able to travel to offices in a five-county area: Wayne, Lincoln, Mason, Cabell and Lawrence County (OH)
Duties:
The Float Office Float is responsible for general office and clerical functions as well as interacting with the public through phone and in-office contact at all Hospice of Huntington, Inc. satellite offices and other duties at the main office. This position will be scheduled at any of the Hospice of Huntington offices, as needed by her/his supervisor. The Float Office Assistant initiates the opening and closing of the office during business hours; turns off the answering service at the beginning of the business day and activates the answering service at the close of each business day; disseminates the mail; receives, logs in, and notifies staff of mail or packages received; maintains administrative files. This position will also be responsible for cleaning the satellite office from which he/she works. Traveling will also be necessary to meet the staffing needs of the five-county areas where we have offices. The Office Float will assist the Human Resources Department as assigned.
Minimum Qualifications:
- Must have reliable transportation
- Must be able to travel to offices in a five-county area
- Must be proficient in computer software such as Word, Excel, PowerPoint
- Demonstrates time management and organizational skills
- Demonstrates highly effective communication skills orally and in writing
- Demonstrates the ability to handle multiple tasks
- Demonstrates the ability to work with little supervision
Education & Work Experience:
- Associate degree or comparable experience; four-year degree preferred
