Duties:

  • Evidence of clinical experience and skills pertinent to assessing and counseling bereaved individuals through individual and group counseling.
  • Evidence of public speaking ability.

Duties:

(TLC Caregiver) performs in-home services that are all non-medical services to individuals who require assistance with daily living activities. The following is a list of services that TLC Caregivers may routinely perform; however, it is not exclusive of other reasonable requests:

  • Responsible for communicating all schedule changes, concerns, and any other care related issues directly to their supervisor.
  • Responsible for documenting in accordance with the company documentation system set forth.
  • Caregiver is expected to interact with patient and in social settings with high integrity, politeness, respect and excellent communication.
  • Caregiver is expected to be prepared to act in emergency situations as trained.
  • Caregiver is expected to communicate with supervisor regarding patient needs and recommendations for care plan change.
  • Caregiver is expected to perform the duties that are outlined in each Care Plan.

DUTIES:

  • Assesses the patient’s nutritional and hydration needs when the patient or family have concerns about food intake.
  • Participates with the interdisciplinary team by providing information on nutritional requirements, diet modification, and support for those patients identified with concerns.
  • Provides information and suggestions regarding increased nutritional needs as appropriate.
  • Provides supportive care for those patients not willing to ingest food or whose ability to absorb food is altered.
  • Educates patients/families about diminished nutritional requirements as a natural part of the last stages of life.
  • Supervises dietary staff in conjunction with the Hospice House Director.
  • Monitors patient response to nutritional therapy.
  • Reviews appropriateness of the dietary therapeutic regimen and the route of administration as indicated or needed.
  • Provides written information about nutritional assessments/reviews for patients/families, hospice employees, MADC participants, or to the physician involved with the patient’s care
  • Provides consultant services for the MADC on a regular scheduled basis and maintains a record of the frequency, nature and durations of visits and/or services.
  • Attends all multi-disciplinary meetings and subsequent meetings if services are provided by the dietitian or recommended by physician/nurse.
  • Completes quarterly and six month assessments/reassessments to evaluate the progress made toward the individuals identified goals with the MADC participant. POC is updated as needed or desired by participant.
  • Other job duties as assigned

POSITION QUALIFICATIONS:

  • Registered dietitian; licensed in the state of WV
  • Ability to communicate effectively both orally and in writing.
  • Philosophy compatible with the hospice concept.

  • License to practice by the West Virginia Board of Examiners for Licensed Practical Nurses.  Must also possess license in Ohio or application must be in progress.
  • Minimum of 1-year patient care experience; hospice or home health preferred.

  • Current driver’s license, automobile insurance, and reliable transportation.

  • Demonstrates flexibility in scheduling.

  • Social work evaluation of patient/family psych/social and environmental needs
  • Implements the social work treatment plan and documents consistent with insurance guidelines in their homes or facilities
  • Assists the primary nurse with establishing and maintaining a treatment plan for each patient/family.
  • Provides the psychosocial needs of the hospice patient/family.
  • Assesses the needs of the bereaved client through verbal and face-to-face interviews.
  • Provides end-of-life bereavement supportive services for patients and their families
  • Understands and discusses available community resources with the client for possible referrals.
  • Participates in community education on grief and bereavement issues.
  • Provides support to selected patients post discharge.
  • Represents the philosophy and services of Hospice of Huntington, Inc.

POSITION QUALIFICATIONS:

  • MSW or BSW with appropriate proof of licensure/certification
  • Licensed in WV and Ohio
    • Demonstrate evidence of knowledge in understanding grief, bereavement, interpersonal issues and family dynamics
    • Valid driver’s license along with an insured, reliable vehicle required.
    • Minimum of one year experience in health care, with a preference in Hospice/Home Health Care.
