Full-Time RN Admissions Nurse

Duties:

  • Provides information about hospice program to potential hospice patients and referral sources.
  • Assesses eligibilty of potential hospice patients in collaboration with the interdisciplinary team.
  • Initiates the physical assessment and plan of care for each patient admission.
  • Serves as a back up to primary hospice nurses for patient visits.
  • Provides teaching to patients/ families to assist in the patients’ comfort as needed.
  • Provides emotional support to patients/ families in a manner that fosters independence and affirms existing strengths.
  • Communicates with team effectively regarding patient care through timely documentation and data entry as well as verbal report.
  • Complies with policies and procedures of Hospice of Huntington, Inc.

Position Qualifications:

  • Graduate of an accredited professional school of nursing; BSN preferred.
  • Licensed registered nurse in West Virginia and Ohio.
  • Minimum of 2 years registered nursing experience; hospice nursing preferred
  • Reliable transportation
  • Liability insurance required.

Registered Nurse – RN Float

Full-Time 

* Must have flexibility as evening and weekend hours may be required
* May be scheduled to work in either the Home Program or at Emogene Dolin Jones Hospice House.

Duties:

  • Perform assessment of assigned patients and derive nursing diagnoses based on assessment data.
  • Plan nursing interventions in collaboration with the interdisciplinary team to resolve current problems or prevent potential problems.
  • Provide teaching to patients/families to assist in the patients’ comfort as needed.
  • Provide emotional support to patients/families in a manner that fosters independence and affirms existing strengths.
  • Assess the need for home health aide services.
  • Assess and provide pain management strategies to assist with the patient’s comfort.
  • Promote working in a team environment.
  • Comply with policies and procedures of Hospice of Huntington, Inc.
  • Protects the confidentiality of private health care information of patients.
  • Promote the philosophy and services of Hospice of Huntington, Inc.

Position Qualifications:

  • Graduate of an accredited professional school of nursing; BSN preferred.
  • Licensed registered nurse in West Virginia and Ohio.
  • Minimum of 2 years nursing experience; home health or hospice nursing preferred.

RN After Hours* Full-Time Float / Admissions for Home Care

  • Assess the need for home health aide services.
  • Assess and provide pain management strategies to assist with the patient’s comfort.
