Full – Time Registered Nurse – RN Float

Duties:

  • Perform assessment of assigned patients and derive nursing diagnoses based on assessment data.
  • Plan nursing interventions in collaboration with the interdisciplinary team to resolve current problems or prevent potential problems.
  • Provide teaching to patients/families to assist in the patients’ comfort as needed.
  • Provide emotional support to patients/families in a manner that fosters independence and affirms existing strengths.
  • Assess the need for home health aide services.

Full-Time RN Team Manager
Mason County/Gallia County*

Duties:

  • Perform assessment of assigned patients and derive nursing diagnosis based on assessment data

Full-time Hospice Aide for Hospice House

Duties:

  • Performs and/or assists the patient with personal care activities as prescribed on the Hospice Aide plan of care.
  • Performs and/or assists the patient/family with light housekeeping and meal preparation.
  • Reports patient status changes to the interdisciplinary team as needed.
  • Maintains current and accurate documentation.
  • Complies with policies and procedures of Hospice of Huntington, Inc.
  • Protects the confidentiality of private health care information of patients.
  • Represents the services and philosophy of Hospice of Huntington, Inc.

Personal Qualifications:

  • High school graduate is preferred.
  • Proof of nursing assistant certification.
  • Minimum of 1-year experience preferred.
  • Current driver’s license, automobile insurance, and reliable transportation.
  • Philosophy compatible with the hospice concept.

Part-time Housekeeping and Cook Position at EDJHH
20-24 hours

Duties:

  • Perform housekeeping and cleaning activities within well-established guidelines.
  • Have mechanical skills in order to assist with general maintenance of facility.
  • Help maintain facility grounds.
  • Prepare and plate food for patients
  • Keep kitchen in a neat, clean and healthy working environment.
  • Receive food and stock, monitor deliveries, place food or stock in the proper areas.
  • Complete daily cleaning and detailed cleaning of kitchens and kitchen equipment.
  • Follow the guidelines set by the Cabell County Health Department.
  • Perform other duties as assigned.

Personal Qualifications:

  • High school graduate or GED preferred.
  • Ability to communicate effectively.
  • Previous similar work experience in a health care setting desirable but not required.
  • Must be able to deal with patients, family members, visitors, public and Hospice staff under varying conditions with a positive and professional attitude.
  • Must possess a current Cabell County Food Handlers Card.
  • Must have reliable transportation.
  • Must be able to read and follow directions and recipes.
  • Must be able to monitor food quality and temperatures to be within the critical control points.
  • Must be able to lift items up to 50lbs.
  • Must be able to work with high temperature foods and equipment.
  • Must work with sharp utensils and equipment.
  • Philosophy must be compatible with the hospice concept

Full-Time Float RN
10-Hour Shifts | 10am-8pm (Evenings and Weekends)

Duties:

  • Perform assessment of assigned patients and derive nursing diagnoses based on assessment data.
  • Plan nursing interventions in collaboration with the interdisciplinary team to resolve current problems or prevent potential problems.
  • Provide teaching to patients/families to assist in the patients’ comfort as needed.
  • Provide emotional support to patients/families in a manner that fosters independence and affirms existing strengths.
  • Assess the need for Hospice Aide services.
  • Assess and provide pain management strategies to assist with the patient’s comfort.
  • Serve as a back up to primary hospice nurses for patient visits.
  • Communicate with team effectively regarding patient care through timely documentation and data entry as well as verbal report.

Personal Qualifications:

  • Graduate of an accredited professional school of nursing; BSN preferred.
  • Licensed registered nurse in West Virginia and Ohio.
  • Minimum of 2 years registered nursing experience; hospice nursing preferred

Full-Time Bereavement Social Worker

Duties:

  • Social work evaluation of patient/family psych/social and environmental needs
  • Implements the social work treatment plan and documents consistent with insurance guidelines in their homes or facilities
  • Assists the primary nurse with establishing and maintaining a treatment plan for each patient/family.
  • Assesses the needs of the bereaved client through verbal and face-to-face interviews.
  • Provides support to clients through individual and group counseling.
  • Understands and discusses available community resources with the client for possible referrals.
  • Participates in community education on grief and bereavement issues.
  • Provides support to selected patients post discharge.
  • Represents the philosophy and services of Hospice of Huntington, Inc.

Personal Qualifications:

  • MSW or BSW with appropriate proof of licensure/certification
  • Licensed in WV and Ohio
  • Demonstrate evidence of knowledge in understanding grief, bereavement, interpersonal issues and family dynamics
  • Valid driver’s license along with an insured, reliable vehicle required.
  • Minimum of one year experience in health care, with a preference in Hospice/Home Health Care.

Duties:

Full-Time and Part-Time Admissions Clerk Position
(Weekend availablitiy is required, as this job will include some weekends)

The Admissions Clerk responds to referral calls from family and community resources to facilitate the referral and admission process; records complete and accurate information in the computer. In addition to these duties, distributes referral information to appropriate staff, refers all medical questions to the correct roles and possesses excellent time management and attention to detail skills. Very fast-paced environment that requires excellent multi-tasking skills.

Duties:

  • Responds to referral calls from family and community resources
  • Records complete and accurate information in the computer
  • Contacts physician’s office to verify referral,  and requests pertinent medical information to support the diagnosis
  • Tracks medical information that has been requested
  • Requests patient information that has not been received and notifies appropriate roles of any problems
  • Communicates with patients/family members to discuss and obtain necessary documentation.
  • Communicates as instructed with referral sources and care partners to provide continuity of communication related to the referral.
  • Assists in data collection.
  • Contact private insurance company to certify insurance coverage and case management to verify hospice coverage; completes corresponding paperwork
  • Copies and distributes referral information to appropriate staff
  • Refers all medical questions to appropriate roles
  • Meets  with families to educate on hospice services as needed
  • Manages the status of all referrals and the follow up needed
  • Works  at the EDJHH as needed
  • Other job duties as assigned

Personal Qualifications:

  • Minimum of 1-year patient care experience; hospice or home health preferred
  • Minimum of one year health care or customer service experience with demonstrated ability to establish good working relationships in person and by phone
  • Valid driver’s license and automobile insurance, and reliable transportation
  • Experience working with computer applications including Windows systems and the Internet
  • Knowledge of Medicare, Medicaid, and Private Insurance required
  • Ability to work in a quick paced, time sensitive environment
