Full-time CNA in the Home Care Program*

We serve our patients wherever they call home, including assisted living, nursing home, or their personal residence.

  • Performs and/or assists the patient with personal care activities as prescribed on the Hospice Aide plan of care.
  • Performs and/or assists the patient/family with light housekeeping and meal preparation.
  • Reports patient status changes to the interdisciplinary team as needed.
  • Maintains current and accurate documentation.
  • Complies with policies and procedures of Hospice of Huntington, Inc.

Full-time RN Team Manager in Mason County*

*(Rotating call is a component of this position)

Duties:

  • Perform assessment of assigned patients and derive nursing diagnoses based on assessment data.
  • Plan nursing interventions in collaboration with the interdisciplinary team to resolve current problems or prevent potential problems.
  • Provide teaching to patients/families to assist in the patient’s comfort as needed.
  • Provide emotional support to patients/families in a manner that fosters independence and affirms existing strengths.
  • Assess the need for home health aide services.
  • Assess and provide pain management strategies to assist with the patient’s comfort.
  • Promote working in a team environment.
  • Provide management of team to ensure continuity of care for Hospice patients and families.

Position Qualifications:

  • Graduate of an accredited professional school of nursing; BSN preferred.
  • Licensed registered nurse in West Virginia and Ohio.
  • Minimum of 2 years nursing experience; home health or hospice nursing preferred.
  • Proof of liability insurance; Proof of reliable transportation.

Per-Diem TLC Caregiver

Duties:

(TLC Caregiver) performs in-home services that are all non-medical services to individuals who require assistance with daily living activities. The following is a list of services that TLC Caregivers may routinely perform; however, it is not exclusive of other reasonable requests:

  • Responsible for communicating all schedule changes, concerns, and any other care related issues directly to their supervisor.
  • Responsible for documenting in accordance with the company documentation system set forth.
  • Caregiver is expected to interact with patient and in social settings with high integrity, politeness, respect and excellent communication.
  • Caregiver is expected to be prepared to act in emergency situations as trained.
  • Caregiver is expected to communicate with supervisor regarding patient needs and recommendations for care plan change.
  • Caregiver is expected to perform the duties that are outlined in each Care Plan.
    • Companionship
    • Light housekeeping
    • Meal planning and preparation
    • Bathing/Showering assistance
    • Personal hygiene
  • Assisting with grooming needs such as hair care (excluding cutting/trimming), shaving with an electric razor, cleaning teeth and routine nail care such as cleaning and polishing (excludes trimming or filing).
  • Errands & shopping
  • Medication Reminders
  • Pet Care
  • Supervise home maintenance such as housecleaning
  • Other job duties as assigned

Education and Work Experience:

  • High School graduate, preferred
  • Proof of current nursing assistant certification, if applicable
  • Minimum one (1) year experience, preferred
  • Valid driver’s license and automobile insurance, and reliable transportation with clean and smoke-free interior.

Part-Time Float Office Assistant

(Must be able to travel to offices in a five-county area)

Duties:

The Float Office Assistant is responsible for general office and clerical functions as well as interacting with the public through phone and in-office contact at all Hospice of Huntington, Inc. satellite offices and other duties at the main office.  This position will be scheduled at any of the Hospice of Huntington offices, as needed by her/his supervisor. The Float Office Assistant initiates the opening and closing of the office during business hours; turns off the answering service at the beginning of the business day and activates the answering service at the close of each business day; disseminates the mail; receives, logs in, and notifies staff of mail or packages received; maintains administrative files. This position will also be responsible for cleaning the satellite office from which he/she works. Traveling will also be necessary to meet the staffing needs of the five-county areas in which we have offices. The Office Float will assist the Human Resources Department as assigned.

Education and Work Experience:

  • Associate degree or comparable experience; four-year degree, preferred

Minimum Qualifications:

  • Must have reliable transportation
  • Must be able to travel to offices in a five-county area
  • Must be proficient in computer software such as Word, Excel, PowerPoint
  • Demonstrates time management and organizational skills
  • Demonstrates highly effective communication skills orally and in writing
  • Demonstrates the ability to handle multiple tasks
  • Demonstrates the ability to work with little supervision

Part-time (24 hours) CNA for Hospice House

Duties:

Full-time (30 hours) Cook at Hospice Care Inpatient Unit

  • Three 10 hours shifts per week
  • Rotating weekends
  • Some Holiday work required

Duties:

  • Prepare and plate food for patients
  • Keep kitchen in a neat, clean and healthy working environment.
  • Receive food and stock, monitor deliveries, place food or stock in the proper areas.
  • Complete daily cleaning and detailed cleaning of kitchens and kitchen equipment.
  • Follow the guidelines set by the Cabell County Health Department.
  • Perform other duties as assigned.

Position Qualifications:

  • High school graduate or GED preferred.
  • Ability to communicate effectively.
  • Previous similar work experience in a health care setting desirable but not required.
  • Must be able to deal with patients, family members, visitors, public and Hospice staff under varying conditions with a positive and professional attitude.
  • Must possess a current Cabell County Food Handlers Card.
  • Must have reliable transportation.
  • Must be able to read and follow directions and recipes.
  • Must be able to monitor food quality and temperatures to be within the critical control points.
  • Must be able to lift items up to 50 lbs.
  • Must be able to work with high temperature foods and equipment.
  • Must work with sharp utensils and equipment.
  • Philosophy must be compatible with the hospice concept.
