Full-time CNA in the Home Care Program*

We serve our patients wherever they call home, including assisted living, nursing home, or their personal residence.

  • Performs and/or assists the patient with personal care activities as prescribed on the Hospice Aide plan of care.
  • Performs and/or assists the patient/family with light housekeeping and meal preparation.
  • Reports patient status changes to the interdisciplinary team as needed.
  • Maintains current and accurate documentation.

Full-time RN Team Manager in Mason County*

*(Rotating call is a component of this position)

Duties:

  • Perform assessment of assigned patients and derive nursing diagnoses based on assessment data.
  • Plan nursing interventions in collaboration with the interdisciplinary team to resolve current problems or prevent potential problems.
  • Provide teaching to patients/families to assist in the patient’s comfort as needed.
  • Provide emotional support to patients/families in a manner that fosters independence and affirms existing strengths.
  • Assess the need for home health aide services.
  • Assess and provide pain management strategies to assist with the patient’s comfort.
  • Promote working in a team environment.
  • Provide management of team to ensure continuity of care for Hospice patients and families.

Position Qualifications:

  • Graduate of an accredited professional school of nursing; BSN preferred.
  • Licensed registered nurse in West Virginia and Ohio.
  • Minimum of 2 years nursing experience; home health or hospice nursing preferred.
  • Proof of liability insurance; Proof of reliable transportation.

Per-Diem TLC Caregiver

Duties:

(TLC Caregiver) performs in-home services that are all non-medical services to individuals who require assistance with daily living activities. The following is a list of services that TLC Caregivers may routinely perform; however, it is not exclusive of other reasonable requests:

  • Responsible for communicating all schedule changes, concerns, and any other care related issues directly to their supervisor.
  • Responsible for documenting in accordance with the company documentation system set forth.
  • Caregiver is expected to interact with patient and in social settings with high integrity, politeness, respect and excellent communication.
  • Caregiver is expected to be prepared to act in emergency situations as trained.
  • Caregiver is expected to communicate with supervisor regarding patient needs and recommendations for care plan change.
  • Caregiver is expected to perform the duties that are outlined in each Care Plan.
    • Companionship
    • Light housekeeping
    • Meal planning and preparation
    • Bathing/Showering assistance
    • Personal hygiene
  • Assisting with grooming needs such as hair care (excluding cutting/trimming), shaving with an electric razor, cleaning teeth and routine nail care such as cleaning and polishing (excludes trimming or filing).
  • Errands & shopping
  • Medication Reminders
  • Pet Care
  • Supervise home maintenance such as housecleaning
  • Other job duties as assigned

Education and Work Experience:

  • High School graduate, preferred
  • Proof of current nursing assistant certification, if applicable
  • Minimum one (1) year experience, preferred
  • Valid driver’s license and automobile insurance, and reliable transportation with clean and smoke-free interior.

Registered Nurse – Emogene Dolin Jones Hospice House

Full-Time (36 Hours)                        Shift: 7pm-7am

Duties:

  • Perform assessment of assigned patients and derive nursing diagnoses based on assessment data.

  • Provide teaching to patients/families to assist in the patients’ comfort as needed.

Inpatient Facility Care Coordinator -Emogene Dolin Jones Hospice House

Duties:

The Inpatient Facility Care Coordinator is responsible for the overall planning, delivery and evaluation of quality hospice care for the Emogene Dolin Jones Hospice House (EDJHH).  The Coordinator is responsible to ensure the inpatient unit is operating within all state and federal regulations. The Coordinator is responsible to communicate and work with members of the HOH management and leadership team for activities related to the day-to-day operations and management that includes, but is not limited to, the health and safety of the facility, scheduling and supervision of the staff and compliance with state and federal regulations. The Coordinator will establish and maintain good working relationships with staff, patients, families, community members and community partners.  The Coordinator will provide direct patient care as scheduled or needed including weekend and or night coverage.

Position Qualifications:

  • Experienced Registered Nurse with Bachelor’s Degree, masters degree in field or related field,  preferred
  • Minimum of 3 or more years inpatient facility experience , hospice experience preferred
  • CHPN, preferred
  • Demonstrates effective leadership ability
  • Illustrates effective use of time management and organizational skills to perform multiple tasks
  • Communicates effectively both orally and in writing
  • Philosophy compatible with the hospice concept.
  • Demonstrates an understanding of various regulatory guidelines (ethical, confidentiality, legal requirements, etc)
  • Demonstrates ability to utilize various computer software systems i.e. Microsoft Word, WordPerfect, etc.
