Duties:
(TLC Caregiver) performs in-home services that are all non-medical services to individuals who require assistance with daily living activities. The following is a list of services that TLC Caregivers may routinely perform; however, it is not exclusive of other reasonable requests:
- Responsible for communicating all schedule changes, concerns, and any other care related issues directly to their supervisor.
- Responsible for documenting in accordance with the company documentation system set forth.
- Caregiver is expected to interact with patient and in social settings with high integrity, politeness, respect and excellent communication.
- Caregiver is expected to be prepared to act in emergency situations as trained.
- Caregiver is expected to communicate with supervisor regarding patient needs and recommendations for care plan change.
- Caregiver is expected to perform the duties that are outlined in each Care Plan.
- Companionship
- Light housekeeping
- Meal planning and preparation
- Bathing/Showering assistance
- Personal hygiene
- Assisting with grooming needs such as hair care (excluding cutting/trimming), shaving with an electric razor, cleaning teeth and routine nail care such as cleaning and polishing (excludes trimming or filing).
- Errands & shopping
- Medication Reminders
- Pet Care
- Supervise home maintenance such as housecleaning
- Other job duties as assigned

Duties:
- The LPN provides full range of hospice nursing care to patients and families in the Home Care Program under the supervision of a hospice RN as a supplement to the care provided by the RN.
- Provides personal care such as bathing
- Monitors patients’ medical conditions and provides accurate reports to attending physicians as appropriate and to the patients’ RN.
- Works in an interdisciplinary care setting to teach, assist, and support patient/family with the dying process.
- Covers On-Call as needed
- Carries out the pain management and symptom management plan to keep the patient as comfortable as possible.
- Reassesses pain and notifies the registered nurse when pain is not manageable on the current medications.
- Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
- Provides care within all service areas of Hospice of Huntington, Inc.
Minimum Qualifications:
- License to practice by the West Virginia Board of Examiners for Licensed Practical Nurses. Must also possess license in Ohio or application must be in progress.
- Minimum of 1-year patient care experience; hospice or home health preferred.
- Proof of liability insurance.
- Current driver’s license, automobile insurance, and reliable transportation.
- Philosophy is compatible with the hospice concept.
- Demonstrates flexibility in scheduling.
- Possesses the ability to travel to all of Hospice of Huntington’s service area.

The following is a list of services that MADC Caregiver may routinely perform; however, it is not exclusive of other reasonable requests:
- Assists participants with the following:
- personal care, including dressing, grooming, and personal hygiene
- use of special aids
- accident preventions
- activities of daily living
- Monitors and supervises participants
- Assists participants at mealtime, including feeding participants as needed
- Assists and encourage activity involvement and provide individualized activities as needed
- May be required to work in the Homecare Caregiving Program
- Other job duties as assigned
Education and Work Experience:
- Minimum of 10th grade education; high school graduate or GED preferred
- Minimum one (1) year full-time or part-time paid or volunteer experience working with adults in a health care or social service setting, required
- Must have reliable transportation
- Must be able to read and write

- Provide emotional support to patients/families in a manner that fosters independence and affirms existing strengths.
- Assess the need for home health aide services.
- Assess and provide pain management strategies to assist with the patient’s comfort.
- Promote working in a team environment.
- Comply with policies and procedures of Hospice of Huntington, Inc.
- Protects the confidentiality of private health care information of patients.
- Promote the philosophy and services of Hospice of Huntington, Inc.
Personal Qualifications:
- Graduate of an accredited professional school of nursing; BSN preferred.
- Licensed registered nurse in West Virginia and Ohio.
- Minimum of 2 years nursing experience; home health or hospice nursing preferred.
- Proof of liability insurance.
- Proof of reliable transportation.
- Philosophy compatible with the hospice concept.
