Full-Time Admissions and Palliative Care LPN

(Weekend and Holiday Availability is required, as this job will include some weekends and holidays)

The LPN will work in an office environment and will work in the field as scheduled. This position requires experience working with Medicare, Medicaid and insurance providers and knowledge of the hospice benefit and state requirements. Must have the ability to adapt from the office environment to working in the field. If needed will act in the LPN and see patients as directed.   Excellent customer service skills are necessary both on the phone and in person. Must have good computer skills, be organized, be a self-starter and be able to problem solve.

Very fast-paced environment that requires excellent multi-tasking skills.

Duties:

  • Communicates with patients/family members to discuss and obtain necessary documentation.
  • Contacts private insurance company to certify insurance coverage and case management to verify hospice and/or palliative care coverage; completes corresponding paperwork.
  • Responds to referral calls from family and community resources to facilitate the referral and admission process; records complete and accurate information in the computer.
  • Possesses excellent time management and attention to detail skills.

Position Qualifications:

  • License to practice by the West Virginia Board of Examiners for Licensed Practical Nurses.  Must also possess license in Ohio or application must be in progress.
  • Minimum of one year’s health care or customer service experience with demonstrated ability to establish good working relationships in person and by phone
  • Minimum of one experience working with computer applications including Windows systems and the Internet
  • Knowledge of Medicare, Medicaid, and Private Insurance preferred; Medicare hospice benefit as well  as state requirements for both OH & WV
  • Knowledge of insurance and medical terminology, preferred
  • Proof of liability insurance.
  • Current driver’s license, automobile insurance, and reliable transportation.
  • Demonstrates flexibility in scheduling as this job will include some weekends and holidays)
  • Possesses the ability to travel to all of Hospice of Huntington’s service area.

Full-Time Admissions RN (12 hour shifts)

This job will require the candidate to work evenings, weekends, and holidays.

Duties:

  • The Admissions RN provides information about hospice programs to potential hospice patients/family members and referral services; assesses eligibility of potential hospice patients in collaboration with the interdisciplinary plan; initiates the physical assessment and plan of care for each patient admission; serves as a backup to primary hospice nurses for patient visits; provides teaching to patients/families to assist in the patient’s comfort as needed,  and provides emotional supports to patients and families in a manner that fosters independence and affirms existing strengths.
  • The Admissions RN will prepare charts for admission to hospice.
  • The Admissions RN works in conjunction with the Homecare staff to provide tuck-in service, perform comprehensive visits, serve as a backup to primary hospice nurses for patient visits and other job duties as assigned/needed during their shift. Communicates with team effectively regarding patient care through timely documentation and data entry as well as verbal report. The Admission RN complies with policies and procedures of Hospice of Huntington, Inc.
  • The Admission RN will assist at the EDJHH as assigned/needed during their shift.   The Admissions RN works an assigned schedule that includes evenings, weekends, and holidays.

Position Qualifications:

  • Graduate of an accredited professional school of nursing; BSN preferred.
  • Licensed registered nurse in West Virginia and Ohio.
  • Minimum of 2 years registered nursing experience; hospice nursing preferred

Part-time (24 hours) CNA for Hospice House

Duties:

  • Performs and/or assists the patient with personal care activities as prescribed on the Hospice Aide plan of care.
  • Performs and/or assists the patient/family with light housekeeping and meal preparation.
  • Reports patient status changes to the interdisciplinary team as needed.
  • Maintains current and accurate documentation.
  • Complies with policies and procedures of Hospice of Huntington, Inc.
  • Protects the confidentiality of private health care information of patients.
  • Represents the services and philosophy of Hospice of Huntington, Inc.

Position Qualifications:

  • High school graduate is preferred.
  • Proof of nursing assistant certification.
  • Minimum of 1-year experience preferred.
  • Current driver’s license, automobile insurance, and reliable transportation.
  • Philosophy compatible with the hospice concept.

Full-time (30 hours) Cook at Hospice Care Inpatient Unit

  • Three 10 hours shifts per week
  • Rotating weekends
  • Some Holiday work required

Duties:

  • Prepare and plate food for patients
  • Keep kitchen in a neat, clean and healthy working environment.
  • Receive food and stock, monitor deliveries, place food or stock in the proper areas.
  • Complete daily cleaning and detailed cleaning of kitchens and kitchen equipment.
  • Follow the guidelines set by the Cabell County Health Department.
  • Perform other duties as assigned.

Position Qualifications:

  • High school graduate or GED preferred.
  • Ability to communicate effectively.
  • Previous similar work experience in a health care setting desirable but not required.
  • Must be able to deal with patients, family members, visitors, public and Hospice staff under varying conditions with a positive and professional attitude.
  • Must possess a current Cabell County Food Handlers Card.
  • Must have reliable transportation.
  • Must be able to read and follow directions and recipes.
  • Must be able to monitor food quality and temperatures to be within the critical control points.
  • Must be able to lift items up to 50 lbs.
  • Must be able to work with high temperature foods and equipment.
  • Must work with sharp utensils and equipment.
  • Philosophy must be compatible with the hospice concept.
