When a loved one is facing a life-limiting illness, we understand that calling hospice for care is one of the hardest decisions you have to make. We also know that from caring for over 20,000 patients throughout the Tri-State Region, most families wish they had called us sooner.

Contacting Hospice does not mean you’re giving up or that you’re relinquishing control over your loved one’s care. We work together with the patient, their family and their existing medical provider(s), to provide specialized comfort-oriented care that helps control illness symptoms, maximize quality of life and alleviate caregiver burden. 

Making a Referral

Referrals to Hospice of Huntington can be made by the patient, a family member, friend, or healthcare professional. Referrals can be made by calling (304) 529-4217 or (800) 788-5480.

Once a referral is made, a Hospice team member will gather necessary information about the patient and schedule a visit to meet with the patient and/or family to discuss next steps.

Eligibility

In order to qualify for hospice care, the patients must meet the following conditions:

• The patient is diagnosed with a terminal illness and given a prognosis of six
• months or less to live if the disease follows its normal course. The prognosis
• must be certified by the attending physicians and the hospice physician.
• The resident chooses comfort/palliative care instead of curative treatments.

Where We Serve

We provide care wherever the patient currently calls home. This includes in their own residence, a family member/friend’s residence, senior living community, assisted living facility, nursing home, hospital or at the Emogene Dolin Jones Hospice House.

We serve families in Cabell, Lincoln, Mason and Wayne counties in West Virginia, as well as Lawrence, Meigs and Gallia counties in Southern Ohio. Click here to view our office locations.
